Delivery SystemTransformation Committee Grant Applications

InterCommunity Health Network Coordinated Care Organization (IHN-CCO) is accepting applications from interested community partners to fund innovative proposals focused on improving health equity and outcomes across Benton, Lincoln and Linn counties through our Delivery System Transformation (DST) Committee. Local health care providers, agencies and community-based organizations are invited to submit their grant proposals by no later than 11:59 p.m. on Thursday, June 25. To be considered, all proposals must address diversity, inclusion, anti-racism and equity (IDARE) along with one or more of the three priority areas identified in IHN-CCO’s 2024-28 Community Health Improvement Plan, including:

1. Access to affordable housing

2. Access to quality care

3. Behavioral health

“Our community partners are essential to improving health outcomes for everyone in our

region,” said Bruce Butler, chief executive officer of IHN-CCO. “Through the Delivery System

Transformation Committee, we are investing in community-led, equity-centered projects that

address some of the most pressing needs identified by our local leaders. We look forward to

supporting proposals that create long-lasting and meaningful improvements for our members and the communities we serve.”

Grants will be awarded between two separate funding pathways:

New partner engagement funding is available for new partners to the DST Committee, and those led by priority populations as identified by the Oregon Health Authority are strongly encouraged to apply. Priority populations include communities of color, tribes, immigrant and refugee communities, migrant and seasonal farmworkers, individuals and families with low income, people with disabilities, and LGBTQIA2S+ communities. These applicants will be prioritized for funding. The goal is for new partners to get to know the DST, participate in DST meetings and workgroups and provide additional support to carry out small-scale projects.

These funds are available for larger-scale proposals focused on innovation and health care

system transformation. Pilot projects should either be a new idea, an existing idea that would be new to our region, or an existing idea that would be carried out in a new, creative way. They should take on the challenges of changing systems and breaking down systemic barriers. The DST Committee is committed to supporting projects that promote community-driven and community-focused approaches to health and well-being by elevating the lived experiences and ideas of communities facing health disparities caused by systemic oppression.

Please note that applicants are not permitted to apply for both Pilot Project Funding and New

Partner Engagement Funding at the same time. We will accept only one proposal per applicant. IHN-CCO will host a virtual question and answer session on Wednesday, June 3 from 2 to 3 p.m. on Zoom for any interested partners. Registration is required, and Spanish interpretation will be available.
